Your Netball Queensland membership (playing or non-playing) enables you to access the Netball Australia National Insurance Program, which includes Personal Accident Insurance Coverage.

For injuries to players during games where the sports trainer attended, CHDNA has already submitted your injury report to Netball Qld.

CHDNA strongly suggests players do not return to play after an injury until a medical clearance is obtained.

Insurance enquiries should be directed to the insurer or Netball Queensland

  1. Download the Claim Form here: Netball Australia Personal Injury Claim Form and complete each section.
  2. Email the form to the Association play@cannonhillnetball.com for authorisation.  The Club will complete the relevant checks, sign and return the claim form to you.
  3. Send the authorised Personal Accident claim form and supporting documentation to Berkshire Hathaway Specialty Insurance Company (BHSI) – ahclaimsaustralia@bhspecialty.com.  Their claims team will confirm receipt of your claim form and advise whether they require further information.
