Scope of Cover
Insurance cover is applicable to all registered and financial players, members, officials, umpires, committee and voluntary workers of Cannon Hill Districts Netball Association Inc (CHDNA) for injury as result of accident whilst involved in any official activity of the Association including games, training, fundraising and meeting.
For injuries to players during games, CHDNA will provide an injury report from the sports trainer if available to support your claim. Only players who are financial members and have signed on for the game at the time of injury are covered by the insurance.
CHDNA strongly suggests players do not return to play after an injury until a medical clearance is obtained.
Claim Procedure
- Email CHDNA (play@cannonhillnetball.com) for a copy of your injury report and membership number if you haven’t already received it
- Please complete Section A and B of this claim form
- Ask Your doctor to complete Section E
- A full printed copy along of your claim must be signed by an executive committee member (Section D) who are available at 6:30pm on fixture nights unless otherwise arranged
- Member to submit claim to insurance
Claims should be submitted within 30 days of the injury

85% of Non-Medicare Medical Costs to a maximum of $1,500 ($50 excess)
Weekly Loss of Income Benefit is $250 week with a 7 day excess for a maximum of 52 weeks
